The Cardiac Surgery Clinic at the Heart and Brain Center of Excellence participated with 4 scientific reports at the jubilee conference of the Medical University – Pleven

The Cardiac Surgery Clinic at the Heart and Brain Center of Excellence in Pleven, led by Prof. h.c. Dr. Vladimir Danov, MD, PhD, took part in the jubilee scientific conference held on the occasion of the 45th anniversary of the Medical University – Pleven.

The four reports are of high scientific value and support the strategy for a long-term follow-up of patients receiving treatment at the Heart and Brain Hospital, as well as the inclusion of new patients in long-term programs for access to advanced treatment.

Heart and Brain Center of Excellence’s team performed a unique operation

Cardiac surgical team at Heart and Brain Center of Excellence in Pleven performed a unique mitral valve reconstruction by implanting artificial chords and stabilizing the valve ring. With such a complete endoscopic intervention, the 79 years old patient recovered significantly faster and less painfully.

A Patient With a Torn Aorta Was Operated in Burgas

An elderly patient was operated in emergency in the Bugras with a torn aorta using the so called “Invasive Intervention”. This happened for a first time at the Multi-Profile Cardio-Hospital for active treatment at the City.

The Aorta of the 74-years old lady patient was disrupted in two places and the invasive intervention was held with emergency. This method has no alternative and Irina Maksakova`s Life was saved due to its execution. Only two hours after the manipulation, the patient breathes on her own and is fully conscious.

Irina Maksakova says: “When am I going to a disco? In a day or two I am ready to go.”

The Head of the cardiology clinic noted that the disrupted aorta part is covered with internal stents and the manipulation was performed via a small cut at the thigh area.

Prof. Vladimir Danov, Head of he Clinic: “This method is already present within our usual clinical practice. With it we are closing a full spectrum of the cardio-surgical operations, as well as for the minimal invasive cario interventions, which are common and a standard practice in Europe and around the world.

A Unique Cardiac Surgery was performed at the “Heart and Brain” Hospital in Burgas

The Cardiac surgical team of the Hospital performed a mini-invasive replacement of the mitral valve of a 72-years old patient.

The patient was admitted as an emergency patient. He visited the “Heart and Brain” hospital with complaints of shortness of breath, palpitations, mild fatigue and comorbid conditions such as diabetes mellitus and gout. He suffered for a long time of arterial hypertonia (high blood pressure) and occasional atrial fibrillation – also known as irregular heartbeat. After a consultation with a cardiac surgeon, cardiologist and a resuscitator and performance of an interventional test and echo-cardiographic examination, the conclusion was that the patient suffered from severe mitral insufficiency. As a next step, the team decided to urgently operate.

The team for the operation was led by Petаr Uzunov M.D. and assistant M.D. Krasimira Karakasheva. The surgery, which took four hours was successful. At the moment, the patient is healing nicely.

The Team decided to execute a mini-invasive heart operation , which is considered to be the first one in the region. The main specific is the application of a special conserving technique that does not allow 20-centimeters long cut in the patient`s chest, as it is being done via the classic procedures. The patient is recovering quickly and is in less pain than compared to the classic heart operation. Another advantage is that via a mini-invasive intervention, the patients do not need to take an anticoagulant (blood thinner), which is usually related to a significant change in lifestyle.

The mini-invasive procedure is being prescribed only with strict indications, so that unnecessary implications and risks are avoided.

The Heart and Brain Center of Excellence’s team of neurosurgeons in Pleven saved a 64-year-old patient with three brain metastasis using an ultrasound resection

The specialists from the neurosurgery clinic at the Hearth and Brain Center of Excellence in Pleven performed a unique brain surgery. After an epileptic seizure, a 64-year-old patient was brought to the hospital with right-sided hemiparesis and partial motor aphasia. Following a carcinoma of the large intestine, it was established that the patient had three brain metastasis in active zones. After conducting on-site contrast MRI and navigation protocol, under the control of neuronavigated ultrasonography and neuromonitoring, the neurosurgeon doctor Emil Marinchev and his team performed a one-stage complex surgery with minimal surgical incisions (trepanations). Under optical zoom and by using ultrasound resection, all three metastasis were removed completely.

The patient was discharged after seven days post-operative hospital stay with normal neurological status and is successfully recovering.

The neurosurgeons at the “Heart and Brain” – Center of Clinical Excellence in Pleven performed the first minimally invasive surgical stabilization of the sacroiliac joint in the country

At the Neurosurgical Clinic of “Heart and Brain” – Center of Clinical Excellence in Pleven was performed the first minimally invasive surgical stabilization of the sacroiliac joint (SIJ) in Bulgaria, using the latest titanium 3D implants of SI-Bone (USA). The neurosurgeons who performed the surgery – Prof. Nikolay Raynov and Dr. Dimitar Haritonov – emphasized the fact that this stabilization technique is extremely safe, fast, and mild and it allows patients to be mobilized only a few hours after the surgery.

SIJ is the cause of chronic pain in the area of the waist, pelvis and legs in more than 25% of patients with such symptoms, but it is still underestimated in the differential diagnosis of spinal diseases. The SIJ dysfunction is a state of altered mechanics with increase or decrease of the expected normal movement or the presence of pathological movements.

Making the diagnosis “painful arthrosis of the sacroiliac joints” is a challenge for medical professionals, because the reliability and validity of the used clinical trials remain insufficient even today. The imaging diagnosis is able to detect the wear of the SIJ, but not to make the final diagnosis, which has to be confirmed not only by clinical studies but also by diagnostic injections of analgesic medications in the joint.

Within the first in Bulgaria Center for Treatment of Chronic Pain at “Heart and Brain” – Center of Clinical Excellence, Pleven, which is currently being created, a complete diagnosis of the chronic pain in the waist and the legs is carried out and the most suitable treatment is appointed for each case. Inclusion of SIJ in the diagnosis and treatment of this pain will allow to significantly increase the rate of successfully treated patients who have not yet been diagnosed correctly and continue to suffer from chronic pain despite the fact that the spine has been treated or operated repeatedly.
